We present a video of an interesting case of acute small bowel obstruction from internal herniation at the Petersen’s defect after a previous gastric bypass. The obstruction was unusual and interesting because there was marked lymphatic and venous congestion with chylous ascites.

This is a 49 yo super super obese, bedbound, male with hypertension and obstructive sleep apnea undergoing elective endoscopic gastric sleeve procedure. Due to his high risk for cardiopulmonary event and severe central obesity, he was not amendable to surgery at this time.
