Revisional surgery after primary RYGB in the presence of a gastro-gastric fistulae requires surgical repair with or without remnant gastrectomy. We present the case of a 44 year old female with weight regain after laparoscopic RYGB due to a gastro-gastric fistula. The preoperative studies demonstrated a sizable fistula at the level of the gastro-jejunal anastomosis. A remnant gastrectomy, a re-do gastrojejunostomy, was performed laparoscopically with no postoperative complications.
